Job summary

Little Sisters of the Poor, Holy Family Residence in Saint Paul, MN seeks a CNA with restorative care experience for day shift. After training, schedule will include every other weekend and reliable transportation is required.

You will work with the Restorative Care team to deliver prescribed therapies, assist with mobility and ADLs, monitor changes in residents, document procedures, and report concerns promptly while respecting privacy and dignity.

Responsibilities

• Document restorative procedures and monitor residents’ condition and progress.
• Promote independence through mobility and ROM exercises and assist with ADLs.

Major Duties:

• Work closely with the Restorative Care Coordinator/nursing staff and rehabilitative staff to help meet the needs of each resident, immediately communicating all issues and concerns
• Provide assigned residents with restorative care in accordance with established restorative care methods, procedures and techniques
• Remain up-to-date regarding information found in residents’ most current restorative program and care plan
• Recognize symptoms and report changes in residents’ conditions in a timely manner.
• Document restorative care procedures per facility policy
• Work on resident care units when necessary
• Efficiently use all equipment including, but not limited to telephones, call lights, paging systems, mechanical lifts, scales, and all therapy equipment
• Observe, disinfect and immediately report concerns regarding therapy equipment
• Principles of/Comprehensive Restorative Care Program
• Demonstrates an understanding of the principles and techniques of restorative care to attain the resident’s highest level of functioning to promote independence and prevent avoidable decline
• Demonstrate use of assistive devices in restorative care interventions
• Report observations of resident’s condition or change in functional status
• Encourages residents with praise to progress and provides positive feedback, while maintaining the resident’s self-esteem, privacy and confidentiality
• Check plan of care daily for changes in goals, new interventions, discontinued equipment or devices, etc.
• Mobility Programs
• Demonstrates appropriate use of mobility, balance, and strengthening exercises, as well as incorporating these into ADLs
• Incorporates fall prevention strategies into delivery of care and verbalizes factors that contribute to falls and implements strategies to minimize fall risks
• Utilizes good positioning/re-positioning procedures and supportive devices
• Demonstrates proper use of various mechanical lifts per manufacturer’s guidelines, foot boards, handrails, canes, and walkers
• Reinforces good body mechanics during mobility exercises and all transfers
• Range of Motion
• Demonstrates understanding of the safe and appropriate technique for range of motion exercises to prevent contractures: active (AROM), passive (PROM), and active assist (AAROM)
• Understands the importance of assessing pain prior to and after range of motion exercises
• Activities of Daily Living Dressing/Grooming Program
• Demonstrate and use task segmentation in assisting a resident to complete ADL’s and appropriate use of assistive and adaptive equipment
• Demonstrates how to modify the environment to promote the highest level of independence and safety in ADL’s
• Eating/Dining Program
• Demonstrates knowledge of resident-specific goals and use of devices related to a restorative care dining
• Document dietary intake accurately and communicate observations toward progressive planned goals, per facility policy
• Continence Program
• Demonstrates knowledge of the restorative role in bowel/bladder retraining programs and the different types of individualized retraining programs including clothing management, hygiene, and adaptive equipment
• Tracks/documents voiding patterns per facility policy
• Demonstrates proper catheter care and peri care per facility policy and procedure
• Skin Care Program
• Describe measures for the prevention of pressure areas and to prevent complications of bed rest using positioning and exercise
• Communication Program
• Demonstrates strategies for assisting residents with receptive and expressive aphasia
• Provides written communication devices when a resident cannot speak or hear
• Demonstrates ability to use positive effective non-verbal communication techniques of appropriate touch, simple hand gestures, eye contact, facial expressions, and body posture
• Amputation/Prosthetic Care
• Assists in the application, operation, and removal of prosthetic devices including dentures
• Assists in prosthetic care, and monitors skin areas under prostheses for skin changes (skin redness, swelling or breakdown, odor, pain) before and after donning and doffing prostheses
• Demonstrates the use of a slide board transfer
• Pain Management
• Lists non-verbal signs and symptoms suggesting pain
• Promptly reports non-verbal behaviors to the licensed nurse, restorative care coordinator, and/or rehabilitative therapist that could be caused by residents feeling pain/discomfort
• Describe and list comfort measures for residents with pain
• Demonstrates appropriate use of special treatments (hot and cold therapy modalities)
• Safety Issues
• Demonstrate safety in the environment with use and maintenance of equipment
• Demonstrates safety of resident with use of gait belt, non-skid footwear, and observing for uneven surfaces during ambulation, when appropriate
• Documentation
• Documents appropriately per facility policies and procedures for restorative documentation and use of electronic records
