Creative Health Solutions, LLC is seeking a Restorative Nursing Assistant to carry out restorative nursing functions within our facility. The role supports residents to increase or maintain their highest possible functioning level under our care team.

You will perform restorative tasks, monitor progress, document care, and collaborate with the nurse team to adjust plans as needed. A valid ISDH CNA certification in good standing is required, with potential 120-day provisional periods for CNAs

Formación

• High school diploma or GED required.
• ISDH CNA certification in good standing; 120 days provisional for out-of-state CNAs.

Responsabilidades

• Provide direct restorative care and delegated therapy tasks as assigned, ensuring accuracy, safety and timeliness.
• Participate in all aspects of restorative care programming (range of motion, walking, etc.).
• Assist with the Bowel and Bladder Program and scheduled toileting procedures.
• Notify Charge Nurse/Restorative Nurse of resident progress, regression, or refusal to adjust care plans.
• Document services and minutes on designated forms; maintain proper records per policy.
• Maintain safety, infection control and a clean work environment; follow protocols and report issues.
• Perform other duties as assigned by Restorative Nurse or designee.

Educación

C.N.A. certification by ISDH in good standing
