Job summary

Pivot Rehabilitation is seeking a Restorative Aide to support restorative and rehabilitative care for ProCare I-SNP members at Park Valley and Falcon Ridge. The role focuses on safe, compassionate personal care and assisting in restorative tasks under the guidance of the Director of Rehab.

The candidate will perform ROM, ambulation, positioning, and self-feeding as trained, aiming to maximize patient independence while documenting progress and changes to care plans.

Qualifications

• State certification as a Nursing Assistant is mandatory.
• Ability to read, write, and speak the English language.
• Must be available to work six days per week.
• Ability to travel to additional SNF locations within geographical area

Responsibilities

• Provides personal care to patients to ensure safety and comfort; assists with restorative or rehabilitative care.
• Performs restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding, within training scope.
• Promotes independence and communicates status to the Director of Rehab daily.
• Contributes to patient care plans with knowledge of conditions/needs.
• Assists in implementing care plans as directed and documents observations.

The overall purpose of the Restorative Aide position is to support in the provision of restorative and/or rehabilitative care of ProCare I-SNP members as appropriate.

• Provide personal care to patients in a manner conducive to their safety and comfort. Assist in the provision of restorative or rehabilitative care.
• Performs restorative nursing tasks such as ambulation, range of motion, positioning, self-feeding, etc., within the scope of individuals' training.
• Promotes and supports the greatest possible degree of independence for patients.
• Advises Director of Rehab (DOR) of Patient's status daily.
• Contributes knowledge of Patient's conditions/needs to patient care plans.
• Assists in the implementation of care plans as directed within the parameters of established policy and procedure.
• Observes and reports on Patients' conditions and documents observations as required.
