About Pine Haven Nursing and Rehabilitation Center

We are a 120-bed facility located in the beautiful woods of Columbia County. A leading senior care provider in the area, conveniently located near the Taconic State Parkway, offering a peaceful commute.

Position: Restorative Aide (*Must be a CNA)
Job Summary:

Reports To: Restorative Nurse/Charge Nurse

The Restorative Nursing Assistant is a Certified Nursing Assistant with additional training in rehabilitation, providing restorative care to residents. Must motivate residents to participate in their restorative programs and communicate effectively.

Responsibilities Include:
  • Transport residents to and from appointments within the facility.
  • Coordinate restorative programs with other departments to ensure programs are not missed.
  • Follow written instructions, document resident outcomes daily, and communicate changes to supervisors.
  • Implement and provide input on residents' restorative programs after training.
  • Participate in restorative meetings and collaborate with the Therapy Department.
  • Ensure all adaptive equipment is available, clean, and functional.
  • Complete restorative documentation, including quarterly notes.
  • Respond promptly to call bells and alarms.
  • Record vital signs as needed.
  • Report changes in resident condition to appropriate staff.
  • Attend conferences and in-service programs.
  • Follow facility policies, including safety standards.
  • Complete pre-placement physical and background check.
  • Adhere to infection control protocols and complete required training.
  • Maintain regular attendance and perform other duties as assigned.
Physical and Mental Requirements:

Proficiency in reading, writing, and communication; ability to perform nursing judgment; basic computer skills; hearing and vision correction; simple math skills; ability to stand, bend, push, pull, and lift at least 20 lbs.

Schedule:

Monday - Friday, 8 am - 4 pm, with every other weekend required.
