Job summary

Reliant Care Management Company is seeking a Restorative Aide to support residents in maintaining and improving physical function through restorative programs. You will work under nursing and rehabilitation staff to follow individualized care plans and contribute to resident independence.

Qualifications

• Current CNA certification or state facility certification required for Restorative Aide.
• Experience in a skilled nursing or long-term care setting preferred.
• Ability to communicate effectively with residents, families, and staff.
• Ability to safely assist residents with mobility, transfers, and restorative activities.
• Ability to follow individualized care plans and instructions from nursing and rehabilitation staff.

Responsibilities

• Assist residents with restorative nursing programs and activities according to individualized care plans.
• Assist with range-of-motion exercises, ambulation, transfers, positioning, and other restorative activities as directed.
• Encourage residents to maintain independence with activities of daily living.
• Observe residents and report changes in condition, mobility, behavior, or functioning to the appropriate nursing staff.
• Accurately document restorative activities and resident participation.
• Follow all facility policies, procedures, infection-control practices, and safety standards.
• Maintain resident privacy and confidentiality.
• Work cooperatively with nursing, therapy, and other members of the interdisciplinary team.
• Perform other duties as assigned.

Essential Duties and Responsibilities

• Assist residents with restorative nursing programs and activities according to individualized care plans.

• Assist with range-of-motion exercises, ambulation, transfers, positioning, and other restorative activities as directed.

• Encourage residents to maintain independence with activities of daily living.

• Observe residents and report changes in condition, mobility, behavior, or functioning to the appropriate nursing staff.

Required Qualifications

• Must hold and maintain a current, valid certification required by the facility/state for the Restorative Aide position.

• Previous experience in a skilled nursing or long-term care setting preferred.

• Ability to communicate effectively with residents, families, and staff.

• Ability to safely assist residents with mobility, transfers, and restorative activities.

• Ability to follow individualized care plans and instructions from nursing and rehabilitation staff.

Physical Requirements

• Ability to stand, walk, bend, stoop, and assist residents with mobility and transfers.

• Ability to safely perform assigned restorative activities.

• Ability to meet the physical requirements of the position with or without reasonable accommodation.
