Responsibilities

• Assist with Restorative Care Plans – implement individualized care plans focused on rehabilitation and functional independence.
• Promote mobility and function – assist with exercises, mobility, and activities of daily living to improve strength and range of motion.
• Track resident progress – document improvements and progress toward restorative goals.
• Provide therapeutic activities – lead or assist in exercises and activities to maintain or improve abilities.
• Collaborate with nursing and therapy teams – ensure coordinated care for optimal results.
• Maintain accurate records – keep up-to-date documentation of care provided and progress.

Your Responsibilities:

• Assist with Restorative Care Plans – Work with residents to implement individualized care plans focused on rehabilitation and functional independence.
• Promote Mobility & Function – Help residents with exercises, mobility, and activities of daily living to improve strength and range of motion.
• Track Resident Progress – Monitor and document residents’ improvement and progress in their restorative goals.

Qualifications:

• Active Certified Nursing Assistant (CNA) Certification
• Previous Experience in Restorative Nursing or Long-Term Care (Preferred).
