A healthcare organization is looking for a resource support provider to assist members in accessing essential services. This remote role requires a Bachelor's degree and 2-4 years of experience, focusing on developing care plans and community engagement. ***Remote role. Candidate must reside in the state of IL.***
Provides resource support to members and connects members to local resources for various services (e.g., employment, housing, independent living, justice, foster care) based on care/service assessment and plans. Works with leadership to identify the best, most appropriate local resources for member's needs based on industry best practices.
Requires a Bachelor's degree and 2 – 4 years of related experience. Or equivalent experience acquired through accomplishments of applicable knowledge, duties, scope and skill reflective of the level of this position. Pay Range: $22.50 - $38.02 per hour
