Hours | .7FTE (24hours/week); 8:30am - 5:00pm |
Weekend Rotation | Every third weekend |
Holiday Rotation | 2 Holidays a year |
Bonus Eligibility | $6,000 |

The Resource RN works on a collaborative interdisciplinary team including RNCMs, Social Workers, CNAs, Volunteer Coordinators, and Clinical Assistants.

HOW YOU’LL MAKE A DIFFERENCE IN THIS ROLE (ESSENTIAL FUNCTIONS)
The Registered Nurse (RN) Resource covers for any nursing needs throughout a specified Agrace team territory. This role is responsible for patient and family satisfaction through the provision of nursing care including but not limited to case management, making assigned patient visits, and conducting admission visits. Responsibilities include comprehensive assessments, following and updating the plan of care, providing interventions, communicating, and documentation.
