Job Description Job Title: Resource Navigator Coordinator

Reports To: Area Director/Assistant Area Director

Classifications: Full time, Salary, Exempt Schedule: Monday – Friday/Alternate work schedule/Some weekends/evenings

Position Overview:

The Resource Navigator Coordinator (RNC) coordinates, delivers, and oversees all Tier 3 and Tier 4 specific services related to formal connection to mental health, substance abuse, and sex offender services recommended during CRT. This position will be responsible for maintaining formal service connections for timely access to recommended post release care for both direct and indirect services related to mental health, substance abuse, sex offender, and evidenced based resources as outlined by the contract. The RNC reviews all JJIS and Electronic Commitment Packet documents as required. In addition, review of transition required information to include CRT recommendations. This will be the single point of contact for formal service connection and monthly communication through referral and connection to services which will be initiated immediately upon release. Will serve as the DJJ identified circuit liaison for the Community Resource Guide posted on DJJ website.

Essential Duties:
  1. The Coordinator maintains an active network with community agencies for the purpose of identifying community resources, making referrals, and tracking progress made.
  2. The Coordinator reviews screening and assessment outcomes for the Community Assessment Tool (CAT) and recommends appropriate evidenced based delinquency interventions.
  3. If an outside service referral is needed the Coordinator will ensure the youth is connected and referred to the appropriate outside agency for mental health, substance abuse, and sex offender services.
  4. Coordinator will follow up monthly with the service provider to verify participation and engagement.
  5. Will serve as the liaison for communication to and from formal providers and will communicate this with the assigned Aftercare Navigator.
  6. The Coordinator shall ensure that client access to services is efficient and well managed.
  7. This position will provide coaching, professional development, and skill building opportunities for staff regarding connection to appropriate resources for the youth’s individual needs.
  8. Provides support and guidance to the Navigator working directly with youth in the home and community setting.
  9. Provide oversight of mental health and substance abuse emergencies to assist the youth and family through a crisis until they are able to attend a formal appointment.
  10. Assists the Aftercare Resource Navigator assign and deliver evidenced based interventions developmentally appropriate to the youth’s domains, skill level, and resources available in their immediate support network.
  11. The Coordinator will be responsible for establishing relationships with formal systems of care within their circuit to include reviewing, updating, and reporting quarterly to ensure resources are accurate on the Department of Juvenile Justice website.
  12. Responsible for all Evidenced Based Services Module Data Entry
  13. Responsible for checking community resources listed in Community Resource Guide every 90 days and updating as directed.
  14. Point of contact for the circuit on DJJ website and will represent the program and Department professionally and with pride.
  15. Provides guidance and approval of direct service recommendations for resources needed.
  16. Facilitates evidenced based groups as needed. Provides recommendations to address a minimum of 1 risk factor from CAT.
  17. Navigator Coordinator shall ensure that client access to services is timely, efficient, and well managed.
  18. This position will be responsible for the coordination of referrals and networking with providers within the assigned circuit. They will be responsible for actively pursuing partnerships with agencies that provide services to post release youth and families.
  19. Build partnerships and obtain memorandum of understanding with agencies providing services in the counties and circuits assigned. When possible, partnerships will be built to help overcome barriers related to wait lists or delays in service initiation.
  20. Provide guidance in coordinating referrals for Resource Navigators
  21. Track referrals from identification of need, resource mapping, submission of referral, follow up on referral status, schedule appointments, and inform Resource Navigators of all scheduled appointments and providers.
  22. Provide overview of service referrals to navigators for YES Plan Goal development and updates on connection to services during transition meetings.
  23. Assign activities, workbooks, curriculum, or Evidenced Based Services to fill service gaps. Lead Community Resource Mapping activities specific to circuit and county resources.
  24. Will be assigned a case load, when needed, during periods of staff vacancy or based on change in referral trends.
Requirements
Qualifications:
  • Education requirements: Bachelor’s degree in related area of study.
  • Experience requirements: 2 years in the following areas: treatment plan development, community referral coordination, service plan development, collaboration with various agencies, resource development and effective utilization
  • Must pass Level II background screening and preemployment drug screen.
