Job Description

We are seeking someone who is willing to travel for in-person client meetings approximately 1-2 days per week within the state of Indiana. Preference will be given to candidates living in Northern Indiana, including Fort Wayne, South Bend, and Valparaiso areas.

Position Summary:

The Resource Facilitator (RF) supports persons living with brain injury, their families, state agencies, medical and allied health team personnel, employers, and other providers in achieving optimal, timely, and durable outcomes, specifically regarding, but not limited to, effective long-term employment retention. The RF also ensures customer satisfaction and outcome attainment without discrimination based on creed, culture, or financial resources.

The RF serves as an advocate for the person living with a brain injury, their family, and their support team to ensure the program participant’s needs and desires are communicated effectively and appropriately to those involved in care and service provision.

The ideal candidate will possess:

  • Bachelor's degree required; master's degree preferred
  • Case management experience
  • Strong verbal and written communication skills
  • Ability to work remotely
  • Willingness to travel within Indiana to meet clients in-person and attend team meetings

Essential Functions:

  • Assess concerns, needed services, and residual impairments that may interfere with vocational reintegration, and refer for appropriate medical rehabilitation and community-based services
  • Develop comprehensive return-to-work plans integrating vocational and rehabilitation therapy goals, addressing issues through accommodation and compensatory techniques
  • Facilitate access to resources through education and advocacy, helping individuals attain needed services and supports based on established plans
  • Monitor the status of plans and the quality of services, supports, and resources
  • Provide brain injury education and training to persons living with brain injury, their families, employers, coworkers, and community providers
  • Promote TBI-informed care with other providers, including education and training in brain injury accommodations
  • Ensure collaboration and coordination among providers and community resources
  • Participate in continuous quality improvement to meet CARF and other standards

  • Flexibility to work with clients with various impairments
  • Proficiency with computers, internet, and office equipment

Experience:

Experience navigating post-acute private and public support systems and managing barriers from a consumer or advocate perspective. Background in healthcare, rehabilitation, or case management preferred.

Salary: Ranges between $24-27/hr depending on experience.

Work schedule:

This is a remote position, generally working Monday through Friday, 8 a.m. to 4:30 p.m., with flexibility. Occasional weekend or evening hours may be required. Quarterly in-person team meetings are held at Easterseals Crossroads headquarters in Indianapolis, IN, which may require an overnight stay if not local.
