Job Description
We are seeking someone who is willing to travel for in-person client meetings approximately 1-2 days per week within the state of Indiana. Preference will be given to candidates living in Northern Indiana, including Fort Wayne, South Bend, and Valparaiso areas.

Position Summary:
The Resource Facilitator (RF) supports persons living with brain injury, their families, state agencies, medical and allied health team personnel, employers, and other providers in achieving optimal, timely, and durable outcomes, specifically regarding, but not limited to, effective long-term employment retention. The RF also ensures customer satisfaction and outcome attainment without discrimination based on creed, culture, or financial resources.
The RF serves as an advocate for the person living with a brain injury, their family, and their support team to ensure the program participant’s needs and desires are communicated effectively and appropriately to those involved in care and service provision.

Experience:
Experience navigating post-acute private and public support systems and managing barriers from a consumer or advocate perspective. Background in healthcare, rehabilitation, or case management preferred.
Salary: Ranges between $24-27/hr depending on experience.
Work schedule:
This is a remote position, generally working Monday through Friday, 8 a.m. to 4:30 p.m., with flexibility. Occasional weekend or evening hours may be required. Quarterly in-person team meetings are held at Easterseals Crossroads headquarters in Indianapolis, IN, which may require an overnight stay if not local.
