Job summary

Helio Health in Syracuse, NY seeks a Wellness Coordinator (LPN) to lead residential wellness initiatives and supervise medication room operations with staff guidance and regulatory compliance.

The ideal candidate holds an active NYS LPN license, can work independently, and communicates effectively with residents and care teams. The role offers $26–$28 hourly pay, a sign-on incentive up to $4,000, and comprehensive benefits.

Qualifications

• Active NYS LPN license required.
• Valid NYS driver\'s license required.
• Experience with mental health or substance use preferred.

Responsibilities

• Facilitate wellness initiatives, education and training for participants and staff.
• Ensure adherence to appointments and medications, coordinating with healthcare providers.
• Oversee operation of medication rooms and audits per regulations and best practices.
• Collaborate with Medication Specialists and Resident Specialists across sites on Med Room operations.
• Assist with coordination between prescribing physicians, managed care, labs and pharmacies to ensure timely fills/refills.
• Ensure documentation meets regulations and Agency policies; train staff on medication policies.
• Identify health education needs and conduct routine wellness education sessions.
• Address systemic issues to improve service quality in medication programs.
• Other duties as assigned.
