Job summary

TCN Harbor House in Yellow Springs, OH is seeking a Graveyard Shift Resident Counselor who monitors residents, provides clinical duties as needed, assists with daily living tasks, and facilitates groups and activities to support safety and treatment goals.

You will document progress, maintain charts, and ensure adherence to policies. The role requires a valid Ohio driver’s license, CPR/First Aid, and de-escalation training. Shift is full-time and on-site at Harbor House.

Qualifications

• Education: High school diploma or equivalent.
• Experience: Some experience with substance abuse and mental health populations preferred; competencies required.
• Certification: CDCA or above; SWA or above preferred; CPR; First Aid; AEGIS (De-escalation); Medication Monitoring.
• Other: Valid Ohio driver’s license; proof of automobile insurance; meet ODADAS licensure/certification requirements; competencies for working with SUD/mental health disorders.

Job Shift: Graveyard

Position Summary: This position is responsible for monitoring clients of the residential facility to ensure client safety and the integrity of the program; providing clinical duties as needed (group and/or individual sessions); assisting with the tasks of daily living; facilitating recreational activities; and completing clinical/facility documentation.

Responsibilities

• Resident Support
  • Assist in designing and participating in resident activities.
  • Identify resident treatment needs and provide appropriate interventions under supervisor direction.
  • Facilitate educational and therapeutic groups and other activities as needed.
  • Reinforce residents’ orientation to house rules, safety procedures, and household expectations.
  • Utilize residential plans of care tailored to each resident.
  • Monitor self-assisted medication administration.
  • Oversee residents’ general health status.
  • Ensure facilities are clean and meet the standards set by Ohio MHAS and TCN.
  • Respect and protect residents’ rights.
  • Complete safety procedures and drills according to agency schedules and standards.
  • Transport clients as needed using company or approved personal vehicles.
  • Accompany clients to appointments, meetings, and other locations, ensuring continuous monitoring.
  • Read, comprehend, and enforce client rules without exception, escalating violations to supervisors.
• Documentation and Reporting
  • Document client progress and activities in residential charts and daily shift reports.
  • Assist in preparing facility summary reports.
  • Communicate effectively with community support personnel, supervisors, and clinicians.
  • Ensure 100% accuracy in documentation and reports.
  • Complete progress notes at the end of each session and submit all documentation within 24 hours.
• Compliance with Standards
  • Provide only services authorized by Utilization Management.
  • Ensure Individualized Service Plans and Progress Notes reflect client input and are signed by clients when clinically appropriate.
  • Maintain accurate and thorough clinical charts for all clients.
• Conflict and Crisis Management
  • Handle crises calmly and effectively.
  • Mediate resident conflicts to promote a respectful and peaceful living environment.
• Collaboration and Communication
  • Maintain open communication with clients, staff, and stakeholders.
  • Foster positive collaboration with other social service agencies.
  • Stay informed about community referral sources.
• Training and Policy Adherence
  • Complete all required annual training, including client rights, safety, de-escalation, and infection control.
  • Adhere to organizational policies and demonstrate behavioral and ethical compliance.

Minimum Qualifications

• Other: Valid Ohio driver’s license; proof of automobile insurance; meet ODADAS licensure and certification requirements; demonstrated competencies for working with persons with substance use and/or mental health disorders.

Knowledge:

• Lifting to 25 pounds, standing, bending, stooping, writing legibly or use of computer, ability to read, and communicate information clearly, ability to recognize unusual or threatening conditions, and ability to remain patient and understanding when working with individuals in various stages of recovery.
