Qualifications

• Must be 18 years or older.
• HS Diploma or GED.
• 1–2 years of employment with the same establishment or agency preferred.
• Criminal background checks and fingerprinting required.
• Valid driver’s license with fewer than 4 points.

Responsibilities

• Assist with activities of daily living including hygiene, meals, and medication.
• Support health and service plans; monitor and report health changes.
• Perform household chores and assist with activities of choice.
• Ensure safety; transport residents as needed and drive vehicle.
• Prepare progress notes on medication administration and activities.
• Engage with residents’ families to facilitate community connections.
