Job summary

Qualifications

Responsibilities

• Plan and monitor monthly budgets, schedules, and goals.
• Maintain and update Individual Service Plans (ISPs).
• Coordinate and attend medical appointments as needed.
• Document progress and prepare monthly reports for coordinators.
• Maintain confidential participant records and correspondence.
• Oversee safety, medication administration, and first aid supplies.
• Act as liaison with families, case managers, and providers.
