Responsibilities

• Support residents in achieving individualized recovery and treatment goals.
• Assist with daily living skills, personal growth, and community integration.
• Participate in the development and implementation of individualized service plans.
• Maintain accurate documentation, progress notes, and resident records.
• Monitor resident progress and communicate observations to the treatment team.
• Facilitate recreational, educational, wellness, and skill-building activities.
• Provide transportation assistance when needed.
