Responsibilities

• Provide case management and access to housing, mental health, and social services.
• Develop individualized service plans with clients addressing housing, employment and life skills.
• Coordinate with providers and community resources; transport clients as needed.
• Document assessments, plans, progress notes and outcomes in records.
• Deliver life skills training and crisis intervention when necessary.
