Job summary
Eliot Community Human Services, Inc. is seeking a Residential Registered Nurse (RN) to join our Developmental Disabilities division in Massachusetts. The role focuses on person-centered care for adults with intellectual and developmental disabilities, coordinating medical care and delivering health education.

Qualifications
- Associates degree in Nursing or equivalent.
- Experience working with adults with intellectual or developmental disabilities.
- Valid Massachusetts driver's license.
- Must possess a valid Massachusetts driver’s license and maintain a personally owned vehicle.
Responsibilities
- Provide direct medical care as ordered by a physician
- Educate and consult with individuals and staff on medical conditions, medications, and general health topics
- Deliver individualized training and facilitate health and wellness groups
- Assist individuals in accessing and utilizing community health resources
- Guide individuals in understanding their medical conditions including symptoms, complications, and treatment options
- Review medical documentation and provide care coordination and consultation as needed
- Coordinate and follow up on medical care and attend appointments as assigned
- Provide side‑by‑side interventions focusing on health education and skills training
- Support the team in assessing symptoms and treatment effectiveness and recommend appropriate actions
- Build relationships with community medical providers and VNA services to enhance care coordination and act as a liaison between individuals, providers, and the treatment team
