PALS OHIO, LLC. is seeking an In-Home Care Specialist to provide person-centered support to individuals with developmental disabilities in home and community settings.

You will implement Individual Service Plans and assist with daily living activities, transportation, and health-related tasks while documenting progress. You will maintain safety standards, participate in staff meetings, and collaborate with the interdisciplinary team to promote independence and quality of life for those served.

Qualifications

• High School diploma or GED required.
• At least 1 year of healthcare experience or experience with individuals with disabilities.
• Current Ohio driver's license with acceptable driving record.
• Criminal background check clearance required.
• Ability to work flexible schedule and be a team player.
• Strong verbal and written communication skills.

Responsibilities

• Safely provide transportation as needed.
• Supervise and support individuals in daily activities.
• Complete documentation including transportation, billing, and program records.
• Attend regular staff meetings and contribute to team.
• Model company guidelines and people-first language.
• Maintain CPR, First Aid, Crisis Intervention, Behavior Support Plan and other training.
• Participate in ongoing training and self-development.
• Collaborate with interdisciplinary team and caregivers.

The In-Home Care Specialist provides continuous and active involvement, which includes an assertive and consistent implementation of an Individuals Service Plan, based on each person’s strengths and needs. The In-Home Care Specialist is accountable and responsible for the direct supervision and teaching- and “hands on” assisting with each person served, including but not limited to, the following areas: homemaker & personal care services, health & medical care, dietary & special menu orders, independent living skill development, vocational and community employment interests, adaptive equipment and assistance supplies/items, mobility and safety awareness, transportation, socialization & leisure skills. The Direct Support Staff is required to function as a team member with co-workers, the interdisciplinary team, and staff associates in an accountable and professional manner.
