PALS OHIO, LLC. is seeking an In-Home Care Specialist to provide direct support to individuals with disabilities in home and community settings in Columbus, OH.

Responsibilities include implementing Individual Service Plans, supervision, documentation, transportation, and skill development across daily living, health, and community activities. Candidates should have at least a high school diploma, a valid Ohio driver's license, strong communication, teamwork, and a commitment to person-centered

• Minimum of High School diploma or GED.
• Current Ohio Driver's License with acceptable driving record.
• Criminal Background verification with acceptable clearance.
• Willingness to work a flexible schedule.
• Ability to work as a team member and communicate effectively with persons served and staff.
• Demonstrates proficiency in verbal and written skills.
• At least 1 year of healthcare experience or working with individuals with disabilities.
• Maintains current and annual training requirements.

The In-Home Care Specialist provides continuous and active involvement, which includes an assertive and consistent implementation of an Individuals Service Plan, based on each person’s strengths and needs. The In-Home Care Specialist is accountable and responsible for the direct supervision and teaching- and “hands on” assisting with each person served, including but not limited to, the following areas: homemaker & personal care services, health & medical care, dietary & special menu orders, independent living skill development, vocational and community employment interests, adaptive equipment and assistance supplies/items, mobility and safety awareness, transportation, socialization & leisure skills. The Direct Support Staff is required to function as a team member with co-workers, the interdisciplinary team, and staff associates in an accountable and professional manner. The In-Home Care Specialist will be expected to implement all job requirements in the home and/or community setting.POSITION’S ESSENTIAL FUNCTIONSSafely provide transportation as needed.Insure all safety equipment is in good working orderResponsible for Individual supervision/supportResponsible for documentation, including but not limited to, transportation, billing and program documentation.Contributes to and attends regular staff meetings.Promotes a supportive environment by modeling the guidelines of company philosophy, confidentiality, people-first language, empowerment, and fair and reasonable treatment of others.Other:Maintain current certification in CPR, First Aid, Crisis Intervention, Behavior Support Plan, and any other required training.Participated in personal and professional self-development to maintain or enhance work skills that allow for completion of job responsibilities.Other related duties, as requested or necessary.Minimum of High School diploma or GED.Current Ohio Drivers License with acceptable driving record is requiredCriminal Background verification, with acceptable clearance according to applicable rules and requirements.
