Job summary

Achieve is seeking a Direct Support Professional – Residential in Chenango County, NY. This full-time role provides direct care, supervision and support to residents in a residential setting, helping them develop independence in daily living activities and ensuring health and safety.

Responsibilities include assisting with ADLs, supporting independent living skills, budgeting, med administration, and attending team meetings. A NY driver’s license is required and lifting up to 50 lbs is expected.

Qualifications

• High School Diploma or GED is required.
• Experience in human services or with individuals with functional differences preferred.
• Valid NY State driver's license and driving record acceptable to agency's insurer.
• Ability to lift and assist with up to 50 pounds, including transfers and physical support.

Responsibilities

• Assist residents with activities of daily living including bathing, grooming, hygiene and toileting.
• Support development of independent living skills such as cooking, housekeeping, laundry and shopping.
• Support money management such as budgeting and making purchases.
• Administer medications and complete required documentation per agency policy.
• Transport and accompany residents to medical appointments, activities and community outings.
• Maintain a clean, organized living environment and participate in assessments and team meetings.
• Communicate health, safety, and behavioral concerns to supervisors.
• Ensure compliance with OPWDD regulations and stay updated on communications.
