High School Diploma is required.
Valid driver's license (USA).
Experience working with individuals with mental health diagnoses or developmental disabilities or substance use disorders.

Responsibilities

Assess each person’s hopes and goals and provide supports to achieve them.
Help people build relationships in their community and with natural supports.
Assist in community participation aligned with the person’s interests.
Protect rights and promote advocacy and personal safety.
Complete documentation including progress notes, goals, and timesheets.
Support daily home operations: housekeeping, meals, laundry, shopping, and transportation as needed.
Attend meetings and trainings; maintain required certifications.
