Job summary

Centurion Health is hiring a Residential Counselor for the FACT program in California. The role involves conducting structured groups, providing psychoeducation, and linking clients to community services within a secure, supervised setting under the FACT model.

You will support skill-building, daily living activities, and treatment objectives while maintaining documentation and collaborating with the program team. Per-diem schedule includes days, evenings, or nights as needed.

Qualifications

• High school diploma or equivalent.
• Minimum of one year of mental health experience required.
• CNAs welcomed.
• Active CPR certification preferred.
• Must have a valid California driver’s license; good driving record.

Responsibilities

• Conducts structured skills-based groups for clients to address coping skills (time management, goal setting, stress, anger, budgeting).
• Provides psychoeducation for clients, including those deemed incompetent to stand trial, in sessions.
• Helps clients connect to important community services (Medical, Medicare, insurance, volunteer opportunities, school).
• Provides psychoeducation on social skills, personal hygiene, interpersonal communication, conflict resolution, activities of daily living, job skills, transit use.
• Supports client documentation and medication management.
• Other duties as assigned.

The FACT program is a statewide system that provides an unlocked, supervised living environment with 24-hour staffing. FACT's model of care is centralized services with a slow transition to community-based services.Under the direction of the Fact Program Director, the Residential Counselor provides community-based services and monitoring for FACT clients to include judicially committed individuals and mentally disordered offenders.
