Job summary

Mercy Haven, Inc. is seeking a Full Time Residential Counselor for the West Babylon Community Residence program to help adults with severe mental illness gain independence through daily living training, counseling, and crisis management.

Responsibilities include planning, documentation, transport, and coordination with service providers, with a comprehensive benefits package and opportunities for growth.

Qualifications

• High School Diploma or equivalent with related experience required
• Bachelor's Degree in Human Services preferred
• Related experience working with people living with mental illness or other disabilities that require assistance in day to day living highly preferred
• A valid NYS Driver's License in good standing

Responsibilities

• Participate in the planning/evaluation of individual service plans and monitor plan objectives
• Hands-on training and assisting residents in the activities for daily living and documenting interaction
• Responsible for accompanying residents on recreational outings and travel training exercises
• Provide counseling, behavioral intervention, and crisis management
• Assist in the day-to-day management of the community residence
• Notify the Assistant Administrator/Residential Administrator of any medical/psychiatric emergency
• Provide transportation as needed for shopping, medical appointments, etc.
• Write progress notes on residents' activities on a daily basis
• Monitors management of medication
• Advocate for residents' benefits
• Teach residents the skills needed to gain independence
• Coordinate staff contacts with local service providers as needed to assist residents in accessing services and to plan for discharge jointly
• Work with support services on behalf of the residents' best interest
