Job summary

Mercy Haven, Inc. is seeking a Part Time Residential Counselor for our Bayshore Community Residence Program.

The role focuses on helping residents develop independent living skills, delivering hands-on training, and assisting with daily living activities, counseling, and crisis management for adults with severe mental illness. Responsibilities include planning and evaluating service plans, documenting progress, assisting with transportation and outings, and coordinating with local service

Qualifications

• High School Diploma or equivalent with related experience required
• Bachelor's degree in human services preferred
• Related experience working with people living with mental illness or other disabilities that require assistance in day to day living highly preferred
• Physical Requirements: ability to sit, stand, move and lift moderately; valid NYS Driver's License in good standing

Responsibilities

• Participate in the planning/evaluation of individual service plans and monitor plan objectives
• Hands-on training and assisting residents in activities for daily living and documenting interactions
• Provide counseling, behavioral intervention, and crisis management
• Assist in day-to-day management of the community residence
• Write progress notes on residents' activities daily and monitor medication administration
• Coordinate staff contacts with local service providers to assist residents in accessing services and plan for discharge
