Job summary

Responsibilities

• Participates in planning and evaluating service plans.

• Provides transportation for recreational and social activities.
• Delivers counseling and crisis management services.
• Manages daily community residence operations in absence of Senior Counselor.
• Notifies supervisors of emergencies and follows agency procedures.

Job description

Duties and Responsibilities

• Participates in the planning and evaluations of individual service plans and monitors plan objectives.
• Write progress notes on service plan on a monthly basis or more often as needed.
• Trains and assists residents in the activities of daily living.
• Monitors medication.
• Responsible for accompanying residents and providing needed transportation for recreational and social activities.
• Provide counseling, behavioral intervention and crisis management services.
• Assist in day-to-day management of the community residence.
• Responsible for administration of residence in the absence of the Senior Counselor.
• Notifies the appropriate supervisory personnel of any medical, psychiatric, or programmatic emergency. (See Functional Program)
• Responsible for other related activities.
• Provides case management and follow-up services.
• Provides transportation, as needed, for shopping, medical appointments, etc.
• Learns and adheres to all agency policies and procedures on an ongoing basis.
• Accepts and follows through on all other duties and responsibilities as assigned.

Qualification and Competencies

• High School Diploma and five years experience in the Human Service field.
• Or
• Bachelors degree in human service field
• Demonstrates organizational and communication skills which would enable effective interaction with agency staff, consumers/clients/families and the community.
• Critical thinking and sound judgment skills are necessary.
