Responsibilities

• Facilitate ADL skill-building and community integration; supervise daily medication routines, budgeting and self-preservation.
• Participate in multi-disciplinary treatment planning and progress reviews with clients and families.
• Provide and coordinate treatment services; document contact and activities per treatment plans.
• Schedule and facilitate treatment plan meetings; screen for substance use and support related planning.
