Macon Resources, Inc. (MRI) is seeking a Residential Case Manager/QIDP to support individuals in a 24-hour living setting.

  • One year of experience with the developmentally disabled or DD/MI.
  • Must maintain confidentiality of payroll, financial, personnel data per laws and MRI policies.
  • Must adhere to MRI Safety Policies and Procedures.
  • Must complete MRI training requirements.
  • Must obtain first-aid and CPR certification within 60 days of employment and maintain certification.
  • Strong understanding of developmental disabilities, mental illness, active treatment, normalization, inclusion, least restrictive environment, choice, integration, self-empowerment, and functional skill development.
  • Ability to adapt to frequent change and make sound clinical judgments in daily and emergency situations.
  • Effective oral and written communication with the public.
  • Ability to identify and implement creative solutions to problems.
  • Ability to organize and prioritize work.
  • Willingness to work flexible hours.
  • Must have reliable transportation and a valid Illinois driver’s license with a proper MVR; insurance coverage as specified.

Responsibilities

  • Author and direct implementation of the Individual Service Plan (ISP) through monitoring methods and documentation of results.
  • Coordinate enrollment and disenrollment procedures.
  • Represent MRI in the planning of individualized programs and evaluation of service effectiveness as part of the IDT.
  • Ensure services are provided in accordance with local, state, and federal laws, licensure/certification/accreditation standards, and MRI policies.
  • Collaborate with interdisciplinary team to assure quality of care and services.
  • Maintain ongoing communication with persons served, families, and other service organizations.

Job description

Under general direction of the Director of Community Living Services, Residential Case Manager/QIDP provides support to persons served in a 24 hour living arrangement on a daily basis. The Residential Case Manager/QIDP authors and directs implementation of the Individual Service Plan (ISP) through monitoring of methods employed, coordination of services and documentation of results; facilitates enrollment and disenrollment procedures; and as the leader or a member of the interdisciplinary team (IDT) represents Macon Resources, Inc. (MRI) in planning individualized programs, evaluating service effectiveness, and in assuring services are provided in accordance with local, state and federal laws; licensure/certification/accreditation standards; and agency policies and procedures.

Job Qualifications
  • A Bachelor’s Degree in human services or related field. Must meet the requirement of a Qualified Intellectual Disability Professional (QIDP).
  • One year experience with the developmentally disabled and/or DD/MI.
  • Must be able to maintain payroll, financial, personnel, and information related to persons served as confidential and in accordance with the Mental Health and Developmental Disabilities Confidentiality Act and the Policies and Procedures of Macon Resources, Inc.
  • Must be able to adhere to the Safety Policies and Procedures of MRI.
  • Must be able to complete MRI training requirements.
  • Must have or be able to obtain first-aid and CPR certification within 60 days of employment and maintain certification throughout employment.
  • Must have a thorough understanding of developmental disabilities, mental illness, active treatment, principles of normalization, inclusion, least restrictive environment, choice, integration, self-empowerment, and functional skill development.
  • Must be able to adapt to frequent change and perform adequately when confronted with critical or unexpected issues; make sound clinical judgments based on sound reasoning in daily and emergency situations.
  • Must be able to communicate effectively and diplomatically in both oral and written form and be able to meet and work with the public.
  • Must be able to identify and implement creative solutions to problems.
  • Must be able to organize and prioritize work.
  • Must be able to work flexible hours.
  • Must have reliable transportation to conduct agency business and/or to transport persons served as needed; must maintain a valid Illinois Driver’s License and a good Motor Vehicle Record; and must maintain personal auto insurance with minimum coverage of Bodily Injury-Liability per Person of $100,000 and Liability per Accident of $300,000, Property Liability of $100,000; Medical Payments of $5,000; and Uninsured/Underinsured Person of $100,000 and Accident of $300,000.
  • Must be able to meet the following physical capabilities:
    • Lift and carry 50; able to assist in lifting and transferring an individual who is non-ambulatory from sitting to standing; standing to lying; and back to sitting from all positions.
    • Full range of mobility--pull, push, turns, stoop, kneel, reach, and bend.
    • Stand for up to 45 minutes.
    • Vision correctable to 20/20 or sufficient to complete job responsibilities.
    • Exhibit manual dexterity
    • Adequate hearing. 15. Must be able to counsel, advise, teach, and resolve conflicts while directing persons served and families.
    • Must be able to utilize a phone, computer, word processing software, E-mail, and the filing system.
    • Must have a means of notifying his/her supervisor if unable to report to work.
    • Must be able to work cooperatively with persons served, other service organizations, and staff of MRI.Must be able to summarize, synthesize and analyze data/information.
