Qualifications

• At least one year mental health experience with target population; experience in case management and/or residential program required.
• Strong documentation and communication skills.
• Familiarity with confidentiality regulations and hospital policies.

Responsibilities

• Provide case management and housing-related services to residential clients.
• Enter assessments and progress notes into program database.
• Maintain regular personal contact with clients to monitor capacity for independent living.
• Coordinate quarterly collaborative service plans addressing identified needs and goals.
• Ensure timely electronic charting and reporting in accordance with confidentiality regulations.
