Job summary

Qualifications

• At least 1 year mental health experience with target population.
• Experience in case management and/or residential program required.

Responsibilities

• Provide case management and housing-related services to residential clients.
• Enter assessment and progress notes into the program database with timely documentation.
• Develop quarterly collaborative service plans addressing identified needs and goals.
• Maintain regular personal contact with clients to monitor capacity for independent living and need for additional services.
