Job summary

Qualifications

Responsibilities

• Collaborates with consumers to create and implement individualized service plans.
• Monitors consumer progress and revises plans as needed.
• Participates in program development and team meetings.
• Maintains accurate records and documentation in a timely manner.
• Keeps residential facility clean and orderly on a daily basis.
• Attends required trainings and adheres to agency policies.
