Job summary

PrairieCare in Maple Grove, MN, seeks a Registered Nurse for its Residential Program for Adolescents. You will assess daily functioning, monitor residents, and assist in symptom management to ensure high-quality care in a therapeutic setting.

Responsibilities include admissions assessments, medication oversight, care coordination, and crisis intervention. Requires MN RN licensure, strong communication, and ability to work across shifts in a team-based, on-site environment.

Qualifications

• Requires RN licensure and ongoing competence in nursing practices.
• Knowledge of nursing practices and ability to apply clinical judgment.
• Ability to communicate clearly with residents, families, and staff.
• Proficiency with MS Office for documentation and scheduling.

Responsibilities

• Conduct comprehensive nursing assessments upon resident admission and assist in discharge planning within residential programs.
• Track and communicate essential medical information verbally and in documentation.
• Assist in coordinating and scheduling medical appointments for residents.
• Oversee medication-related responsibilities including MAR review and patient education.
• Collaborate with the residential team to maintain a therapeutic environment for clients.
• Build therapeutic rapport with clients and provide basic counseling support.
• Assist clients in practicing daily living skills and respond to crises with de-escalation techniques.
• Contribute to treatment planning by observing behaviors and providing feedback on progress.
• Provide leadership and on-call support to the residential team as needed.
