Fora Health is seeking a full-time Residential Admissions Counselor to provide comprehensive assessments and treatment planning for individuals with substance use and co-occurring disorders. You will work collaboratively with patients, treatment teams, and community partners to create personalized treatment plans. POSITION OVERVIEW
We have an opening for a full-time Residential Admissions Counselor opening at our primary Portland location. The Residential Admissions Counselor position provides professional assessment & initial treatment planning for substance use and co-occurring disorders. Duties and responsibilities include: performing intake assessments that utilize both ASAM and DSM5 criteria for diagnosis and placement at the appropriate level of care; collaboration with patients on the creation of an individualized treatment plan based on assessed needs, goals, and objectives; care coordination with treatment team members, community partners, and referents as needed; and provide group counseling.

WORKING CONDITIONS
HOURS AND PAY
This is a full time position earning $26.68-29.48 per hour (depending upon experience). This position is non-exempt and eligible for overtime compensation.
LOCATION
This position is an on-site position at our SE Cherry Blossom location in Portland, Oregon.
