The role supports development and facilitation of programming, collects data for assessments, and helps implement the care plan to meet individual resident needs. A high school diploma and care-related certifications are preferred, with long-term care experience valued.

Qualifications

• Experience in long term care and wellness programming requirements.
• Federal regulatory qualifications for activities program supervision.
• Certification in BLS and First Aid is preferred.

Responsibilities

• Assists in developing the community wellness programming.
• Facilitates daily events and programming to meet residents' needs.
• Performs data collection to assist with residents' assessment processes.
• Assists in implementing the plan of care based on residents' needs.

High School Diploma or equivalent

BLS Certification

First Aid Certification

Regulatory eligibility for activities program (federal §483.15(f)(2))

2 years of experience in a social or recreational program in healthcare
