Qualifications

• High school diploma/GED and 3 years of job-related experience providing social or direct care services.
• Experience leading a team or supervisory role preferred.
• Background checks and vaccinations may be required.
• Must have valid driver’s license.

Responsibilities

• Arrange for needed medical care and services to ensure medical care is obtained.
• Assist with development of an individual service/treatment plan for each consumer.
• Counsel residents on behavioral problems and daily decision-making.
• Instruct residents on daily living skills in one-on-one or group settings.
• Observe and monitor resident behavior.
• Provide corrective feedback on behavioral problems and decision-making.
• Participate in case reviews and plan continuing services.
• Perform housekeeping duties and errands.
• Plan, shop for, and prepare nutritious meals and assist families.
• Prepare and maintain records of resident progress and services.
• Drive state vehicles to transport clients/residents to appointments.
• Provide transportation for residents to appointments and activities.
• Assist in delivering educational group lectures.
