Job summary

Georgia Department of Behavioral Health and Developmental Disabilities is seeking a Client Support Worker for the Community Integration Home in Columbus. Responsibilities include developing service plans, counseling on behavioral issues, arranging medical care, and providing transportation to appointments and activities.

Qualifications

• High school diploma/GED and one year of job-related experience in social or direct care services.
• Basic computer skills.
• Ability to communicate and interact with mental health professionals in a team environment.
• Ability to observe and record symptoms, reactions, and progress.
• Valid driver’s license.

Responsibilities

• Arranges for needed medical care and services to ensure care is obtained.
• Assists in developing an individual service plan for each resident.
• Counsels residents on behavioral problems and daily decision-making.
• Instructs residents in daily living skills one-on-one or in groups.
• Observes and monitors resident behavior and progress.
• Provides transportation for residents to appointments and activities.
• Plans, shops for, and prepares meals including special diets.
• Maintains records of resident progress and services performed.
