Job summary

Qualifications

• Experience providing social services in a healthcare setting.

Responsibilities

• Develop and execute a comprehensive social services program, offering counseling and support services to residents and families
• Assess and address residents' medical-related social needs
• Coordinate family meetings and facilitate ongoing discussions between residents, families, and the treatment team
• Advocate for residents' rights while liaising between all parties involved
• Manage discharge planning efficiently
• Ensure residents and families receive top-notch service in a compassionate atmosphere that respects individual needs and rights
