Job summary

Cascadia Healthcare in Tacoma, WA seeks a Social Services Assistant to support the Social Services department in daily operations. You will work with residents, families and care teams to outline daily goals and discharge plans, and help obtain services such as Medicaid applications and insurance appeals.

Core requirements include a high school diploma or equivalent and the ability to maintain resident confidentiality.

Qualifications

• Six months experience in a long-term care environment preferred.
• Ability to work cooperatively as a member of a team.
• Knowledge of standards of practice for long-term care social work is a plus.

Responsibilities

• Assists the Social Services department in day-to-day functions.
• Works with residents, families and other team members to outline goals of their day at admission and the plan to meet those goals and discharge as appropriate.
• Assists residents/family members in obtaining appropriate forms to gain services, such as Medicaid applications or insurance appeals, as well as services and/or equipment at discharge.
• Ensures punctuality and regular attendance for assigned shifts.
