This position will coordinate and provide personal care support for an assigned caseload of residents in a way that promotes dignity and recovery. This position is also responsible for administering medications at least 3 shifts per month.

Principal Duties
  • Maintain current medication administration certification and accurately administer resident medications (at least 3 shifts per month).
  • Develop rapport and communicate regularly with all residents to effectively assess needs and provide care in a respectful manner. Use mental health recovery principles to encourage residents to develop their strengths, talents, and set personal goals.
  • Monitor resident activity and document important behavioral issues. Provide crisis management and behavioral interventions as necessary, using a team approach to deal with difficult situations. Assure proper follow-up and documentation on incidents during the shift, notifying the on-call administrator when necessary.
  • Coordinate daily personal care support for assigned residents, including such tasks as monitoring and assisting with personal hygiene, room care, securing transportation, securing and managing medical and behavioral health care, and managing finances.
  • In conjunction with the Resident Care Manager, complete Personal Care Assessments and Support Plans (RASP) for each assigned resident accurately and on time.
  • Effectively use the computer to track resident appointments and care needs and properly document all resident support provided, maintaining resident files according to regulatory guidelines.
  • Provide resident support and care in the areas of personal hygiene, room care, providing transportation, communicating with medical and behavioral health professionals, and managing finances both for assigned residents and other residents as needed.
  • Assist residents with securing any appropriate community services and available financial and insurance benefits such as Medicare, Medicaid, and MH/ID services.
  • Communicate effectively in person and through phone and email.
  • This position requires a professed Christian with a heart for working with mentally and emotionally challenged adults in a Christian residential care setting. Pray regularly for spiritual, emotional, and physical well-being of residents and for your own insight and wisdom.
Schedule

This position has the following full-time schedule of approximately 40 hours per week:

  • Weekday Schedule: Typically Monday – Friday 8:00am – 4:30 pm, including a ½ hour break.
  • Weekends: Every fourth weekend (Saturday & Sunday). A week day off is scheduled for each weekend shift worked.
  • Evenings: Flexibility to work occasional evening shifts (12 – 8:30 PM) or 2nd shift (2:30 – 11:00 PM).
  • On-Call: One on-call weekend shift per month (second shift).
  • Holiday: Assigned to one of 3 rotating holiday teams, further details from supervisor.
QualificationsProfessional Requirements
  • Bachelor’s Degree in Psychology, Social Work or related field; and/or experience working in a similar Human Service agency is preferred.
  • Experience in providing home care, long-term care, and/or experience in a similar Human Service agency is preferred.
  • The job requires a congenial and professional working relationship with others. Must have strong rapport with all visitors, staff, residents, their families, and professionals.
  • Driving a vehicle for business purposes is necessary for this position. That requires that he/she have a valid driver’s license, an acceptable MVR upon hire, and must at all times meet the guidelines in the Personnel Manual for an acceptable driving record.
Physical, Social, Emotional, and Spiritual Requirements
  • Physical: Ability to accomplish the tasks associated with this position is essential. Must frequently lift and/or move 10 pounds, occasionally assist with lifting up to 50 pounds. Must be able to stand for up to 4 hours, reach above and below, kneel, crouch, crawl, and stay generally active throughout the work day.
  • Social: Strong rapport with staff, residents, families, and professionals is essential. All staff are encouraged to attend and participate in picnics and special events.
  • Emotional: Skills of perception, understanding and caring for people are essential. He/she must be patient, calm under pressure, emotionally stable, and not easily-provoked.
  • Spiritual: In a visible role, this position requires spiritual insight, maturity and patience, which comes out of a personal relationship with Jesus Christ.
