Freedom Village of Holland Michigan is seeking a compassionate Registered Nurse (RN) to serve as Resident Services Director. This leadership role combines clinical expertise, resident advocacy, wellness programming, care coordination, and relationship building to promote an exceptional resident experience.

The role emphasizes guiding residents through healthcare decisions, coordinating across disciplines, and leading wellness initiatives to support independence and aging in place.

Qualifications

  • Must be a current RN licensed in Michigan.
  • Three years of management experience in a related senior living or healthcare organization preferred.
  • Excellent communication, relationship-building, and problem-solving abilities.
  • Ability to work collaboratively across multiple disciplines.
  • Experience in senior living, home health, case management, care coordination, wellness coaching, geriatrics, assisted living, or skilled nursing preferred.

Responsibilities

  • Provides leadership in the development of objectives, standards of care, and policies and procedures affecting the resident experience specifically related to health and wellness.
  • Oversees the Wellness Nursing Department in Independent Living.
  • Leads the collaborative care weekly meeting with the multi-disciplinary team to strategize for optimal resident outcomes, campus flow, and occupancy.
  • Leads transition evaluations and aging in place discussions with residents and families when a change in condition necessitates additional services or a move within the continuum of care.
  • Develops a referral network of service providers in and outside the community.
  • Acts as a resource for options and choices related to supportive strategies to improve health, function, and lifestyle.
  • Initiates resident and/or family meetings for issue resolution, risk reduction, and opportunities to enhance general well-being.
  • Assists residents in achieving their health and wellness goals within the 8 Dimensions of Wellness framework.
  • Collaborates to plan, implement and evaluate Wellness Programming and health education opportunities.
  • Pro-actively builds relationships with residents and families, and fosters trust and community culture.
  • Initiates and leads new resident orientation and admissions assessments for independent living residents.
  • Maintains records and ensures confidentiality; participates in leadership meetings and strategic planning.
  • Oversees LCS policies and emergency preparedness programs; supports staff during emergencies.
  • Attends in-service training and education sessions as assigned.
  • Strives for high levels of resident satisfaction with sound judgement and organizational skills.
  • Other duties as assigned by the Executive Director.

It's about helping residents live with purpose, independence, connection, and confidence. We are seeking a compassionate and experienced Registered Nurse (RN) to serve as our next Resident Services Director, a highly visible leadership role that combines clinical expertise, resident advocacy, wellness programming, care coordination, and relationship building. As the Resident Services Director, you will serve as the community's health and wellness leader, helping residents navigate healthcare decisions, transitions, community resources, and supportive services while promoting an exceptional resident experience. This role is ideal for an RN who enjoys building relationships, solving problems, educating others, and helping older adults thrive.


What You’ll Do

This is not a traditional nursing position.


As Resident Services Director, You Won’t Spend Your Days Passing Medications Or Providing Bedside Care. Instead, You’ll Use Your Nursing Expertise To



  • Guide residents through healthcare decisions.

  • Support successful aging and independence.

  • Help residents stay safely in their homes longer.

  • Lead wellness and prevention initiatives.

  • Coordinate care across multiple healthcare settings.

  • Build meaningful relationships with residents and families.

  • Create solutions before small concerns become larger problems.


You will become a trusted advisor, advocate, and leader who helps residents live well and thrive.


Qualifications


  • Current Registered Nurse (RN) license in the state of Michigan.

  • Three years of management experience in a related senior living or healthcare organization preferred.

  • Excellent communication, relationship-building, and problem-solving abilities.

  • Ability to work collaboratively across multiple disciplines.

  • Experience in senior living, home health, case management, care coordination, wellness coaching, geriatrics, assisted living, or skilled nursing preferred.


Principle Duties


  • Provides leadership in the development of objectives, standards of care, and policies and procedures affecting the resident experience specifically related to health and wellness.

  • Oversees the Wellness Nursing Department in Independent Living.

  • Leads the collaborative care weekly meeting. Communicates and collaborates with the multi-disciplinary team to strategize for optimal resident outcomes, campus flow, and occupancy.

  • Leads transition evaluations and aging in place discussions with the resident and family (when applicable) when a change in the resident’s condition necessitates additional services or a physical move within the continuum of care. Ensures that residents are in the appropriate level of care and/or are receiving supportive services to best meet their goals and needs. Partners with the Executive Director to address challenges related to residents at risk.

  • Develops a referral network of service providers in and outside the community.

  • Acts as a resource for options and choices related to supportive strategies to improve health, function, and lifestyle.

  • Initiates resident and/or family (as appropriate) meetings for issue resolution, risk reduction strategies, and opportunities to enhance the general well-being of the resident.

  • Assists residents in achieving their health and wellness goals, within the framework of the 8 Dimensions of Wellness, personal choice, and mind-body-spirit.

  • Collaborates with the Wellness Nurse to oversee post-acute discharge planning and follow-up support.

  • Partners with Community Life Director and Wellness Nurse to plan, implement and evaluate Wellness Programming and health education opportunities.

  • Pro-actively builds relationships with residents and seeks to establish trust, respect, and connection. Communicates effectively with families and staff. Fosters and drives measures to improve community culture.

  • Initiates and leads the new resident orientation program for independent living residents. Performs admissions assessments for prospective residents.

  • Maintains records appropriate for independent living and assures confidentiality of all resident information contained therein.

  • Participates in leadership meetings, responsibilities, and strategic planning. Actively engages in support of global community needs.

  • Oversees LCS Policies including but not limited to: Personal Service Provider policy, Electric Mobility Vehicle Policy, Reasonable Accommodations Policy.

  • Pro-actively assists residents, family, and community to prepare for, and provide support during, emergency situations. Leads the Caregiver Emergency Action Plan program and the registration for Electric Medical Equipment and Evacuation needs.

  • Attends in-service training and educational sessions as assigned.

  • Strives to achieve high levels of resident satisfaction. Uses sound judgement and strong organizational skills.

  • Other duties as assigned by the Executive Director.


Physical Requirements/Working Conditions

Must be able to sit for up to 50% of the workday. Must be able to stand and walk throughout the community for up to 50% of the workday. Must be able to push or pull 50 pounds unassisted. Must be able to lift up to 30 pounds. Must be able to bend and reach. May be exposed to infectious and contagious diseases.
