Job summary

HumanGood in Pennsylvania is seeking a Resident Services Coordinator to organize, implement and evaluate the Social Services program for residents who live independently in the community. The role involves assessing psycho-social needs, maintaining records, and ensuring access to supportive services to promote independence.

The position is full-time, Monday–Friday, 9:00 AM–5:00 PM, with a pay range of $25.00–$27.00 per hour depending on experience.

Qualifications

• 1+ years of experience in case management.
• 2+ years of social services experience, preferably in a senior care setting.
• Bachelor’s Degree in social work or related program such as gerontology, psychology, or recreation therapy.

Responsibilities

• Organize, implement and evaluate the Social Services program for residents.
• Assess resident needs based on admission history, health, psychology and social factors.
• Maintain resident records and ensure linkage to needed supportive services to help residents live independently.

Job description

Our Resident Services Coordinators are passionate, organized, and compassionate towards residents. You would be responsible for organizing, implements and evaluates the Social Services program to meet the psycho-social needs of residents. This may include resident assessment, maintenance of resident records, assuring that residents are linked to the specific supportive services they need to continue living independently in the community.

Schedule: FULL-TIME

• Monday - Friday at 9:00 AM - 5:00 PM

Pay Range: $25.00 - 27.00/hour, depending on experience

To Be Successful In The Role, You Would Have

• 1+ years of experience in case management required.
• 2+ years of social services experience, preferably in a senior care setting
• A Bachelor’s Degree required in social work or related program such as gerontology, psychology, or recreation therapy
• Bilingual - English/Mandarin
• Social Services certification preferred
