Delmar Gardens on the Green is seeking an organized, compassionate Social Services Designee to support our residents and families in a versatile role within the Social Services Department, with backup support to Admissions and the Business Office.

The ideal candidate will coordinate services, assist with admissions, conduct tours, handle admission paperwork, and help with Missouri Medicaid applications and follow-up in a fast-paced facility.

Qualifications

• Experience in social services, Medicaid, or admissions is preferred.
• Strong communication and organizational skills are essential.
• Comfortable managing multiple responsibilities in a fast-paced healthcare setting.
• Experience in long-term care, admissions, social services or Medicaid preferred.

Responsibilities

• Assist residents and families with needs and questions.
• Coordinate services and transportation for residents.
• Support the admissions process and conduct facility tours.
• Assist with admission paperwork and Medicaid-related applications, reinvestigations, and follow-up.
