Responsibilities

• Demonstrates leadership by planning and implementing new programs aligned with Facility goals.
• Receives inquiries from prospective residents and maintains inquiry records.
• Interviews patients or families when admission is requested.
• Explains costs of care for appropriate level of care as determined by physician.
• Acts as liaison between families, discharge planners, social service agencies and physicians.
• Notifies families of admission approvals, bed numbers and arrival times.
• Greets patients and families, assists settling in.
• Chairs Admission Conference/MDS meetings for new admissions.
• Participates in required meetings and quality assurance activities.
• Assists with discharge planning and connects residents to community resources.
