Job summary

The Gardens at West Shore in Camp Hill, PA seeks a compassionate Social Services Director to lead the Social Services Department in our skilled nursing facility. This leadership role advocates for residents’ social and emotional well-being and oversees program compliance and department operations.

You will conduct social assessments, guide admissions, coordinate support for residents and families, document psychosocial status per regulations, and develop volunteer initiatives to enhance resident

Qualifications

• Bachelor’s Degree in Social Work or related field.
• Knowledge of social service practices, laws, regulations, and guidelines governing long-term care.
• Strong communication, documentation, and organizational skills.

Responsibilities

• Develop comprehensive social assessments and individualized care plans, identifying medically related social and emotional needs, realistic goals, and actionable interventions
• Actively participate in pre-admission and admissions processes
• Serve as a compassionate liaison for residents and families, responding professionally and independently to concerns and needs
• Accurately document residents’ behavioral, emotional, and psychosocial status in accordance with regulatory requirements
• Provide individual and group support to residents and families during adjustment periods, crises, or special circumstances
• Establish, coordinate, and expand a volunteer program to support resident social service needs
• Orient new residents and families to the facility and assist with their transition to the community
