Job summary

Cedar Hills Center for Nursing and Rehabilitation is seeking a compassionate Social Worker to support residents and families throughout their healthcare journey.

You will advocate for residents, coordinate services, and help develop individualized care plans in collaboration with an interdisciplinary team. This role emphasizes dignity, independence, and quality of life within long-term care.

Qualifications

• Bachelor's degree in Social Work or related human services field is required.
• Healthcare or long-term care experience is preferred.
• Strong communication, organization, and interpersonal skills.
• Excellent documentation and case management abilities.
• Ability to maintain confidentiality and exercise sound professional judgment.

Responsibilities

• Perform psychosocial assessments and develop care plans.
• Provide counseling and emotional support to residents and families.
• Participate in interdisciplinary care plan meetings.
• Assist with discharge planning and transitions of care.
• Coordinate resources and community services for residents.
• Educate residents and families about available services and rights.
• Maintain accurate, timely, and confidential documentation.
• Build positive relationships with residents, families, staff and providers.
• Advocate for resident rights, dignity and independence.
