Dunbar Center Inc in West Virginia is seeking a Social Services Specialist in a nursing center, requiring WV Social Work License and a passion for improving patient quality of life. You will collaborate with the interdisciplinary team to address social, emotional, and mental health needs and coordinate services under NASW ethics.

You will hold meetings with patients and families, participate in care planning, discharge education, and maintain community referrals to support residents’ well‑being

Qualifications

• Bachelor's degree in social work or human services field is required.
• Must possess any certifications/licensures as required by State of employment to practice in long‑term care.

Responsibilities

• Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well‑being.
• Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs.
• Hold meetings with patients/residents, families, and significant others as requested and to provide support and information to improve quality of life.
• Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers.
• Establish relationships and maintain contact and referral flow with community‑based agencies/services that benefit our patient/resident population.

Education
