Job summary

Acts Retirement-Life Communities is seeking a qualified Primary Care Office Nurse (LPN) to support the health and well-being of residents. You will deliver high-quality, person-directed nursing care within the Primary Care Office and collaborate with the Nurse Practitioner and wellness team to promote independence and thriving in place.

The ideal candidate holds an active LPN license in good standing, at least two years of clinical nursing experience, and CPR/AED/First Aid certification.

Qualifications

• Active LPN license in good standing with the state Board of Nursing.
• Minimum of two years of clinical nursing experience, preferably in senior living or wellness.
• CPR, AED and first aid certification required or obtained within three months.
• Experience in office management or care coordination is strongly preferred.

Acts is currently seeking qualified candidates for our Primary Care Office Nurse (LPN) position.

In this role, you will be responsible for supporting the overall health and well-being of residents by delivering high-quality, person-directed care and nursing services within the Primary Care Office. This role is essential in advancing the Acts Signature Well-Being Model by contributing to interdisciplinary care, supporting proactive health monitoring, and facilitating access to well-being resources. Collaborates closely with the Nurse Practitioner and the broader wellness team to foster a holistic, resident-centered approach that promotes independence, dignity, and thriving in place.

Requirements

The ideal candidate will meet the following requirements:

• Active Licensed Practical Nurse (LPN) license in good standing with the applicable state Board of Nursing
• Minimum of two years of clinical nursing experience, preferably in a senior living, primary care, or wellness-focused environment
• Prior experience in office management, care coordination, or leadership roles is strongly preferred
• Current certification in CPR, AED, and first aid (or obtain within three months of employment)

Pay Range

$24.72 - $31.97 / hour. Starting rate will vary based on skills and experience.
