Qualifications

• ND LPN license (active or temporary) required.
• BLS certification within orientation period.
• LPN program evidenced by licensure.
• Previous nursing home experience preferred, but not required.

Responsibilities

• Assist the RN with assessments, data collection, and continuous monitoring of residents’ physical, emotional, and functional status.
• Provide delegated nursing care, including medication administration, treatments, wound care, and chronic condition monitoring per care plans.
• Document assessments, interventions, and resident responses clearly in the electronic record to support regulatory compliance and continuity of care.
• Communicate changes in resident condition promptly with the RN, provider, and family; update individualized care plans as needed.
• Supervise CNA staff during daily care activities, ensuring proper techniques, safety practices, and resident dignity.
• Maintain a safe, clean, homelike environment by following infection prevention, safety, and equipment handling procedures.
