Job summary

Ciena Healthcare in Michigan seeks a licensed Social Worker to provide psychosocial support to residents and their families, ensure regulatory compliance, and assist with care and discharge planning, PASARR, and advanced directive facilitation. You will coordinate community resources to promote well‑being and quality of life.

Qualifications

• Minimum of bachelors degree in social work or bachelor7s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology.
• One year supervised social work experience in a health care setting.
• Current Social Work license in the state as required by state regulations; LSW/LMSW/LCSW/LBSW/LLBSW/LLMSW preferred.

Responsibilities

• Supervises psychosocial support and services provided by Social Worker and Social Services Designee.
• Carries out supervisory responsibilities in accordance with the organizational policies and applicable laws.
• Interviews, recommend hires and trains employees
• Plans, assigns, and directs workflow.
• Prepares work assignment sheets, taking into consideration resident safety, the educational preparation, experience, knowledge and ability of the persons to whom the assignments are made.
• Receive calls and find replacements for absent staff as needed
• Transfers employees within units as needed.
• Prepares performance evaluations with the understanding that such evaluations impact tenure of probationary employees and wage increases of non-probationary employees.
• Schedules and adjusts lunch and rest breaks.
• Receives and resolves employee complaints.
• Monitors and corrects job performance of employees.
• Adequately supervises assigned staff and assurances that those staff are supervising employees assigned to them.
