iCare Health Network is seeking a Social Worker to coordinate and supervise delivery of social work services for residents, ensuring compliance with state and federal regulations. The role involves interviewing residents, planning discharges, and collaborating with a multidisciplinary team to access community services.

The ideal candidate holds a Bachelor's Degree in Social Work or related field and has at least one year of experience in long-term care providing direct social services.

Formación

• Bachelor’s Degree in Social Work or related field is required.
• Experience in long-term care providing direct social services is preferred.

Responsabilidades

• Conduct face-to-face resident interviews and assess cognitive, mood, behavioral status and other psychosocial factors.
• Participate in planning for resident discharges.
• Respond to clinical emergencies, regulatory visits, surveys and issues outside normal hours as needed.
• Direct and supervise delivery of social services to residents and families.
• Collaborate with the multidisciplinary team to access community-based services.
• Serve as liaison with contracted mental health practitioners and consultants.
• Develop and implement new behavioral and psycho-social service programs.
