Job summary

Qualifications

• High School diploma or equivalency required.
• Social Service Director Certification required.
• One year of supervised social work experience in a health care setting working directly with individuals.

Responsibilities

• Facilitate residents’ safe transitions back into the community through interdisciplinary discharge planning and arrangement of community-based services and follow-up care.
• Oversee Advance Care Planning at admission and review any Advance Directives regularly.
• Ensure staff are aware of each resident’s code status and end-of-life wishes and educate residents and representatives about health care options and ramifications.
• Promote residents’ rights and dignity, and maintain confidentiality of health information.
• Maintain compliance with facility policies, procedures, standards, and regulations; report noncompliance or breaches to appropriate personnel.
• Participate in life safety and emergency drills and fulfill responsibilities during emergency plan activation.
• Interview residents or their representatives to obtain social history and arrange transportation or items needed for daily living.
• Contribute to quality of care through attendance at Care Plan meetings and committee work.
